WSIB and Workplace Forms
Workplace injury assessments and completion of WSIB documentation and employer required forms.
Travel Vaccines
Pre-travel consultations and administration of recommended travel immunizations based on your destination.
TB Testing
One-step and two-step tuberculin skin testing available for employment, immigration, and school requirements.
Insurance and Legal Forms
Completion of insurance forms, disability paperwork, letters of medical necessity, and other third-party documentation.
The services listed above are not covered by OHIP and are billed directly to the patient at private rates. Fees are available at the front desk and will always be communicated to you before any service is provided.
